Abstract This systematic review aims to test the hypothesis that microbleeds detected by MRI are common and show a characteristic pattern in cerebral fat embolism (CFE). Eighty-four papers involving 140 CFE patients were eligible for this based on literature search up 31 January 2020. An additional case was added from hospital records. Patient data individually scrutinised extract epidemiological, clinical imaging variables. Characteristic microbleed resembling “walnut kernel” defined as punctuate hypointensities of monotonous size, diffusely located subcortical white matter, internal capsule corpus callosum, with mostly spared corona radiata non-subcortical centrum semiovale, susceptibility- or T2* weighted imaging. We present a case of cerebral fat embolism (CFE) that demonstrated evidence of diffuse white matter cytotoxic edema on diffusion-weighted magnetic resonance imaging, in addition to punctate hyperintensities on T2-weighted and diffusion-weighted imaging. The case suggests that CFE represents a combination of occlusive arteriolar disease and secondary neurotoxicity.
